How To Report Maintenance Service Request

All tenants are required to complete a maintenance service request form.

The forms are located in the office, laundry room and for after hours a box is located on the front office door.

If you have an emergency, or cannot get to the office to complete a request form, please contact the office at 540-277-2988, or Steve Crim, Maintenance Supervisor at 540-409-7504 or Donna Crim, Office Assistant at 540-247-8939

All maintenance service request are completed in the following priority order:

Emergency maintenance issues

Normal/regular maintenance request are scheduled by the date the request is received

Preventive maintenance request

Please do not stop the maintenance person and verbally request or ask him to take care of your issue.

The maintenance supervisor has a daily schedule of maintenance request to complete and cannot be expected to remember what every tenant tells him. It is the tenant’s responsibility to report all maintenance service issues-If you do not report the issue, we cannot respond to your issue if we do not know about it. There is a process in place and all tenant’s are expected to follow the process.

Remaining Units (MPR Rehab) for Painting and Flooring Replacement

As a result of the MRR Rehab, there are still several units left to paint and complete new flooring in.

Starting in January 2020, we will continue to complete painting and flooring in the remaining units.

For those tenants in the remaining units, if you would like to volunteer for your unit to be done, please contact Donna in the office.

Annual Recertification Tenant Requirements

• Provide proof of all household income (including social security income, SSI disability income, pension/requirement income, wages, child support payments, alimony, unemployment benefits, & DHHR monetary assistance.
• Provide verification of assets
• Complete Verification of Employment form if applies, Eligibility, Income, & Deduction Checklist form-applies to all tenants, and Authorization For The Release of Information form-applies to all tenants.
• Tenants must report all changes in household status that may affect their eligibility (change in household income, household size and members)
